Ghostscript has little to do with a document paper size.

Ghostscript only emulates a Postscript printer but it does not help to write 
the Postscript file. So the application writing the Postscript file needs to 
handle paper sizes itself. (Currently we use QT for all the Postscript 
generation.)

KOffice's paper sizes are those of QT's class QPrinter (QPrinter::PageSize to 
be exact.) All other sizes are supposed to be made with the "Custom" size.

However your idea to have the smaller ISO C envelope sizes at hand is 
interesting. Please add a bug report for a wish at KDE Bugs: 
http://bugs.kde.org
