After considering this a little longer, I have to say this is not a bad 
idea at all. :-)
The alt problem is not that big a problem since we already said all images 
should have the keep ratio enabled by default anyway. 
With that fact I estimate that the keep-ratio will not be used more then 
the disabling of rulers/grid feature in practice.

On top of that is that it is no problem to only press the 'keep ratio' key 
later in the scale/move.

I'll change my list to your suggestion above.
