Not quite sure what your problem is, if I open a KWord document of size 
US-Letter and press print, that is the size my print dialog shows.
If I select a new page of A4, then that is the size shown in the print diag.

Do you think this is incorrect behavior?

I am using the CVS KOffice 1.1 version, together with kprint.
