I'm sorry for the slow reply, i've rather busy with other stuff recently.
Please use the koffice mailinglist for questions for usage, you are sure
to get a much faster response.

Anyway, your question;

I don't know what it is that makes your smart quotes dissapear, maybe your
first check should be to see if the output shows up in the print preview, and
also check that the font used for the quotes (select one) is a scalable font
(make it huge and see if the edges are smooth or really blocky) If it looks
ugly at bigger sizes then you are missing fonts in your installation.

About the inches verses mm, right click the ruler (left and top of the document)
and select inches :)
