You would need to find a way to reliably intercept middle mouse clicks before 
any of the widgets get the clicks. Shouldn't be too hard from there on.

You could see if you can intercept mouseclicks in KApplication::notify, check 
if receiver is a QWidget and then check if receiver->topLevelWidget () is a 
KMainWindow.
