I don't think kmail supports SMTP auth. yet.

File a wishlist bug report in kde bts, if not already requested. The 
workaround i'm using is setting up exim with smtp authentication and sending 
mails to localhost. Unfortunately, postfix doesn't seem to support auth. and 
sendmail is way too 'authentic' to configure. And of course, exim is a pain 
in the *** but I have to use it to get proper auth.
