Good idea. But you'll have some work ahead of you in order to parse the text 
fields so that same-word-variants are grouped together 
("monitor","crt","display","screen" all mean the same thing in english). 
Otherwise you risk making incorrect conclusions based on the subjective data.

Another alternative would be to set up a table with multiple choices for each 
column, with an optional write-in box for each icon. Example:

Action: (-) Undo (-) Go Back (-) Scroll Left (-) Move Cursor Left
Other:  ___________

Doing it in this manner might be a solid way to gain additional information 
about each icon. A dynamic form might also include other people's write-ins 
in subsequent multiple choices for that icon. 

Just a thought.
