Hello Folks,

I created the attached simple form in Quanta and uploaded it to the 10M 
space my ISP provides for my web page. I opened it in my browser and 
filled it in then 'submitted' it. My email client's message composer 
popped up and I hit the 'send' button. I then picked up the message from 
my email account and there was no attachment with the form values.

Can somebody tell me what is wrong with my form please. I am a complete 
novice with forms.
