Under Caldera-2.2 (using kernel 2.2.10), KVT will not start if you are
using Unix 98 pty's.  If you attempt to start kvt, you get the following
error:

kvt: could not open slave tty /dev/pts/3

It should be noted that Konsole works perfectly in the same setup with
Unix 98 pty's.

Additionally, I put a perror() call in after the failed open() of the
slave tty to get some additional information.  The perror call reports
Input/output error when the slave TTY /dev/pts/? (where ? is a number) is
opened.   I attempted to debug things with gdb and couldn't find anything
obvious causing the open() of the slave pty to fail.  The code in this
section is almost identical to the code in Konsole, making it even more
perplexing...

I am using: EGCS 2.91.66, glibc 2.91, Caldera OpenLinux 2.2 with kernel
2.2.10.
