It is rt-extension-spawnlinkedticketinqueue and the offending line is
this one in html/Callbacks/SpawnLinkedTicket/Elements/ShowLinks/Default:
<form action="<% RT->Config->Get('WebPath')
."/Elements/SpawnLinkedTicket" %>" name="SpawnLinkedTicket">

You should check your WebPath setting in RT_SiteConfig.pm.
