"Tools" -> "Data Sources..." 
  "New Data Source" 
	Tab "General"
	    "Database type" : "Text"
	    "Data source URL : browse to whereever your csv file is.
	Tab "Text"
	    File in various obvious flelds

And then in your Form file reference this datasource.
