I should have been more specific: You ALSO need ISO 7816-3 Amendment 1:
Protocol type T=1, asynchronous half duplex block transmission protocol.

section 9 of the
                document the last sentence is "The specification of this
protocol is under
                study and will be included in this clause when available". I
got this
                document in March 98. The date of this document is 1989. Is
there a newer
                revision?
