This dialog is certainly an improvement, even switching the Tab order
alone is an improvement.

Only now to do I finally realise the "Title" in the second dialog was
referring to Mr./Mrs./etc, how confusing.

Having the General Title and Keyword infromation up front is a big
improvement because as far as I am concered that is the interesting
information you actually want to set. (One way or another, the personal
information is something you would only really want to set once and leave
the same for most documents but the title and keywords is more important
because it could be different for every document.)

Maybe it is just your theme and a matter of my own personal taste but I
dont particularly like seeing all those Frameborders.

I could nit pick other details but I would encourage you to take a look at
the properties dialog use by OpenOffice and if you can also look at
Microsoft Office I think you might consider making some futher changes
without even needing me to suggest them (but if I have time tomorrow I
will probably make some other suggestions).
