With the development version of KMail it is no longer possible to create a top level imap folder.

KMail: cvs yesterday
Server: cyrus
Prefix: none
Altnamespace: yes
IMAP-type: online

No ouput to console/gdb. I can create a testaccount if needed or test in an other way. 

Matt Douhan reports it is not possible with dimap either.
