No. All Mailboxes (aka folders) are checked by KMail when you are using a imap 
server.

I don't know what exactly your problem is, but for me it works like this:

Cyrus Imap Server. Messages are filtered by Sieve and stored into many 
mailboxes (around 150 different boxes).
Kmail is configured to check for new mail on this server every minute. And 
wherever my sieve script stores the mail to, KMail finds the new message, no 
matter in which folder it has gotten.
I think this is correct behaviour and if it does not happen to you, there may 
be something wrong with your configuration.
