I have two possible explanations for this:
1.) You didn't close KAddressbook after removing the person from the 
addressbook. For some reason only after closing KAddressbook KMail gets 
the changed address list (at least in KDE 3.0.x).

2.) The address is a recently used address and you didn't disable "Show 
recent addresses" in KMail's Adressbook dialog.
