KEditToolbar only seems to handle automatically updating the toolbar when called by \
the factory() method, not when using actions.  

Since KMail uses the actions method, when configuring the toolbar in either the main \
window or a compose new mail window, pressing the Apply button has no effect.

The way I solved this problem was to added code into the slotEditToolbars() slot of \
both kmcomposewin.cpp and kmmainwin.cpp to connect the KEditToolbar signal \
newToolbarConfig(), which is called whenever OK or Apply is pressed, to a new slot, \
which calls createGUI to update the toolbar.

I'll provide a patch in the next email, as I cannot see a way to attach one from the \
KDE bug report wizard.
