karbon14 1.5.0 has only one option 'Empty document'.

It's good that 'always use this template' is available.

One other thing: the label 'use this template' that _creates_ the doc
using the active template is a bad choice IMHO. The verb 'use' in HCI
systems is used to apply a property to an object.
Computer users have 'connected' the action of creating an oject (doc)
with the word New (or Create).
