Why don't you just use a custom kfiledialog, which embeds the options into
the save-as dialog?

According to
http://developer.kde.org/documentation/library/cvs-api/kio/html/classKFileDialog.html

you can give a custom widget in the constructor of KFielDialog, where you
can put the needed ui elements...

 KFileDialog::KFileDialog  	(const QString &startDir,const QString &filter,
  QWidget *parent, const char *name, bool modal, QWidget *widget)
