First, you need to install the attached icons as hicolor.  That is in:

	$KDEDIR/share/icons/hicolor/<size>/apps/

Note, if you install the toolbar icons in: $KDEDIR/share/apps/<app-name>/pics/" this is 
not so simple and I don't really know if KDE supports more than one theme there.  I will 
look into this.
