Good point.  I will try to make it appear in the same place.

That's more or less by design.  I think MIDI should definitely be a separate 
KCM, in the Sounds and Multimedia section.  I kinda think the user should 
have to do something that causes it to show, like enable MIDI, or run a 
wizard, or check some box in MIDI, so that the MIDI KCM is not setting there 
in the tree.

As I understand it, the Apply and Cancel buttons are supplied by the KCM 
dialog class (not totally sure), so I left em off the UI files.  Having play 
test sound there is a nice idea - it would fit well there.
