Okay if you start dtsession in your Xsession.kde3 after exec startkde and in 
your $HOME/.Xdefauts (create it) add;

Dtsession*waitWmTimeout:	1

dtsession will run along with kde, then create a button that does the command;

dtaction LockDisplay

this works fine, however it doesn't seem to like my smartcard as dtsession 
trys to start it, but dtlogin already has so i get an error and dtsession 
won't start, but it does work with UseSmartCard disabled.

the only real downside it, dtsession starts filemanager, help and a resource 
monitor as well... but i think you'd be able to disable this behaviour in 
your .Xdefaults
