Log Message:
Use strict in every package, which found some possible bugs and made
them easier to find in the future. Now ALL variables must be qualifed in
use vars or with my.
kdoc:
Fixed formats_wanted init.
Fixed reset of class stack.
Use string reference instead of eval. Now output backends will no longer
die silently on error.
NewNamespace and linkNamespaces implemented.

kdocAstUtil.pm:
Added findIn and used it in findRef.
NOTE: findRef still does not use namespaces for ref searches.
Added linkNamespaces.

kdocCxxDocbook.pm:
Added some simple tag generators.
Added printTextItem and image support.

HTML stuff:
moved table generator to HTMLutil and use it for full list generation.

Doc parser:
Parse "Image" tag.
