OK  I ran some tests and then looked in /var/log/daemon.log

TEST 1 - kdm in /etc/inittab with the -debug 255 -daemon options
X Screen came up (in grey) but no greeter, eventually when I killed X 
(ctrl-alt-bksp) and did a pstree I had a kdm with a kdm-config as a child and 
3 other kdm-configs left on their own.  /var/run/xdm.pid was created (and I 
had to manually remove it after killing kdm)

Some key bits of the log (seems to restart kdm-config just after starting X)

TEST 2 - kdm started from inittab but without debug option

Greeter screen came up (screen background blue) and I could try and login - 
tried both as me and as root, and both times I get login failed.  I then 
selected "console" from the menu which got me into a command line where I did 
a telinit 2 to stop kdm from being respawned.  No processes were left 
running, but /var/run/xdm.pid was still there and I had to delete it.

There is an interesting three lines in the log which were also in Test1 
(although more verbose and buried).  These keywords were in the sample kdmrc

TEST 3 - kdm started from the command line with the -debug 255 option

The blue background came up and then immediately disappeared to be replaced 
by the standard X grey. No greeter.  ctrl-alt-bksp kills kdm, but kdm-config 
is left running and has to be killed manually.  /var/run/xdm.pid is left and 
has to be removed manually.

The log was essentially the same as Test 1.

TEST 4 - kdm started from the command line without the -debug option

Greeter screen comes up - and login fails (as per test 2).  Log same as test 
2.

After going to "console" I have to type ctrl-c to kill kdm.  /var/run/xdm.pid 
still there.
