Well kmail won't even start for me because my ~/Mail directory contains an inbox
maildir for mutt. (BUGREPORT)

I think you should let kmail parse the mailto: url format but
let the mail agent be configurable! Please! I would love to be able to use
mailto: links in konqueror / KDE in combination with mutt.

Not everybody likes or uses kmail. (I cant use any mail agent without
threading!)
