This isn't quite right.

In case your display is 8-bit maintoolbars AND toolbars should
use 16x16 by default (as these are locolor).

In any other case 22x22 should be default for toolbars and 
32x32 should be default for maintoolbars (both of these sizes 
are hicolor).
In every case all sizes should still remain configurable
by the user using either the kcontrol-modul (for global
configuration) or using RMB -> Icon sizes -> 16,22,32
to configure the toolbar currently being used.
If 22x22 or 32x32-versions don't exist the 16x16-version should
be displayed (not being scaled unless they are 
being placed in the scale-directory).

IF you develop applications please make sure that 16x16-icons
do exist -- they are "required". All other sizes are 
"recommended".
