From the kedittoolbar docu:
 *   KEditToolbar dlg(actionCollection());
 *   connect(&dlg,SIGNAL(newToolbarConfig()),this,SLOT(slotNewToolbarConfig()));

and reload the plugins in slotNewToolbarConfig().
