Try this:

1) Right-click on an empty spot in the toolbar, and click on "Configure 
Toolbars".
2) In the drop-down box at the top, select "Location Toolbar <Konqueror>".
3) In the "Current Actions" box on the right, ensure that there is an item 
called "Location Bar:".  If it's not there, it should be in the box on the 
left, and you can just click and hit the right-arrow-button.
4) Click OK when done and cross your fingers.

If that didn't work, try going to the Settings menu, click on Toolbars, and 
make sure that the Location Toolbar is enabled.

I run KDE CVS, so some of the items may look different in KDE 3.1.3, but that 
should be the general idea.  Hope it helps.
