Just don't hold it down too long.  Most versions of Windows you have to be 
holding the Shift key at the right time (usually when the CD-ROM drive blinks 
a second time).  I used to hold down the Shift key as soon as I inserted the 
CD-ROM until I was sure the CD-ROM drive wasn't loading, but when I tried XP 
it gave me this very annoying StickyKeys dialog box.  Apparently the 
accessibility features are turned on by default in XP.  Not necessarily bad, 
but it really threw off my rhythm. :(

Now I use KDE, and it's not a problem, however. :)
