For those who use a Help button in the toolbar I need to add a slot that
will trigger the KHelpMenu::appHelpActivated() of the internal KHelpMenu
object of KTMainWindow.

Today the only way a help button in a toolbar can start 
KHelpMenu::appHelpActivated() is to allocate a new KHelpMenu object 
and then connect to its appHelpActivated() slot -  BAD!

So I am going to add a new slot "KTMainWindow::appHelpActivated()" 
which will allocate the internal KHelpMenu object (if not already allocated)
and then call the KHelpMenu::appHelpActivated().
