Pulseaudio supports per-application output mapping. For example you can use
pavucontrol or kmix to change the output of amarok (and only amarok) to HDMI
while the global perference will still be Internal. This per-application
setting then takes preference over the global setting leading to behavior as
outlined where application foo will use another output. In a plasma workspace
session this will usually be indicated by a notification popping up saying that
the output has changed (i.e. it has been changed from the global default to the
application default). This automatic output switching can also be observed when
running amarok --debug-audio. 
On a general note Phonon/Amarok does not actively control the actual output
device when PulseAudio is used, all it does is indicate an audio category
according to which PulseAudio will then choose an appropriate (working) output
device (as reflected by kcmshell4 phonon).
