KDE-look has some documentation. If that is not enough, download a theme, take 
a look at it, use it as a template.

Umm, visual artists will often see the images first. You can always make up a 
concept later, after all :-) If you really want it the other way around, try 
visualization. Close your eyes, try to see an apple. Does not work? Try not 
to see an orange - you probably saw an orange. Now, think of your concept, 
close your eyes, ask for an image that represents your concept. Or again, try 
not to see it. With some training, it will work. 

Some artists wait for inspiration to strike them. Dancers on the other hand 
have performances scheduled. Friday, 9 pm, improvisation. So, they have 
techniques to get in the mood - things like meditation. If you need images, I 
guess visualization is the thing to do.
