Although I have not checked lately, I think this is a known bug. 
A "simple" remedy is:

(NO WARRANTY, DO IT ON YOUR OWN RISK)

1. Move all mails in the folder to a temporary  folder.  
2. Exit kmail.
3. Truncate the file in question e.g, 'cat > "MailFolderName"'  (break the 'cat'
operation with CTRL-C). This will delete all data in a file, so be
careful. 
4. Start kmail
5. Move every message back to your folder.

(and the temporary folder is now empty, but please check its file size as well)

I do this for my "inbox" and "trash" file once in a while.
