That install CD is very very old. It is only a release candidate for
1.4.

Recent CDs that may work live in the
releases/x86/2004.0/livecd/universal.

I'm not sure why this is happening, nor how to fix it.

I think the best solution is either to install from an existing distro
(as documented in the handbook) or from a knoppix CD if you have one of
those knocking around (also documented in the handbook). It may also
work fine with the 2004.0 CD.

I'm sorry I can't offer an actual solution.
