is your modem a plug and pray? do you know what com port it is set to answer on?
to determine if its plug and pray.. boot win95 and let it do its thing.. then
select reboot pc (make sure you dont kill the power)... when LILO loads up and
goes into linux.. see if minicom will work then. If so.. then its plug and pray
wich will require either a warm boot such as you just did.. or using a utility
found at redhat called isapnp. Also 'ls -l /dev/modem' too see where its
pointing. here is the breakdown of com ports. 
 DOS                                                       LINUX
COM 1                                                   /dev/cua0
COM 2                                                   /dev/cua1
COM 3                                                   /dev/cua2
COM 4                                                   /dev/cua3

also sometimes they do something called a COM 5 where they tell the device to
work on a different irq suchas IRQ 5... you can fix that by editing your
/etc/rc.d/rc.local file to include a setserial command 
example:
'/bin/setserial /dev/cua2 irq 5 spd_vhi'

the spd_vhi flag is a good idea on any modem faster than 28.8
