i've debugged the situation, that's what happens :

kcontrol correctly modifies the .kde/share/config/kdeglobals; but if
you simply move or modify a toolbar in any kde application (eg
konqueror), then this application writes in its own config file
a section that contains the toolbar settings, including the
icontext option. From that moment on, this toolbar option cannot
be modified via kcontrol, but only with a right click on the toolbar
itself and selecting the correct option. In other words, after a little
personalization of the most common kde application (kmail, konqueror,
koffice ...) this options in kcontrol becomes pretty useless, because
every application has its own toolbars settings and doesn't follow the setting
in kcontrol.

but afterall it's simply an aesthetical problem :-)
