I had the chance to have a look at a system which showed the same problem.
Relevant part of supportInformation:

Currently Active Effects:
-------------------------
kwin4_effect_fade
kwin4_effect_blur

Unloading the fade effect, made the window go away.

@Thomas: could something trigger an incorrect unref in AnimationEffect?
