I enabled "Write tags in image meta-data as keyword", it works, but if I rename
a tag, it doesn't change the keyword of my images which have the tags.

Is there a way to sync the digikam4.db with the image keywords?

Do you know a command-line tool (for scripting) which can modify keywords?
