when I had the problem it happend with fglrx 8.10, 8.11, 8.12. Xorg was 1.5 qt
was 4.4, plasma was from then current svn (over a period from roughly
september/october to january).

Sometime after that it stopped - but that was with 4.2.X and then current
qt-copy. I didn't retry until 4.2.1 which was ok, AFAIR. The problem didn't
reappear for me from that point on. 4.2.2 was ok for me. current svn for 4.3 is
ok.
