Current adblock patch (v4)

This is the latest version of the patch.

Changes:-
Incorporated David Faure's cleanups.
There's now an option to enable/disable the filter.
Option of replacing ads with "ghostbuster" icon, or stripping out completely.
Right click context menu on images allows block by image or by host.
Moved the options in with other konqhtml files.
General tidying and stupid bugs fixed. :)
