OK, here is a way to consistently reproduce the problem on a CD-ROM drive (in Debian \
konqueror 3.3.1-4):

Preconditions:
* Set "Maximum number of instances kept preloaded" to 1 (Control Centre / KDE \
Components / KDE Performance). [Setting it to 0 gets rid of the bug]:
* Ensure no preloaded konqueror exists: kill all konqueror processes.
* Ensure CD-ROM drive is unmounted.

Do this:
* Click on CD-ROM icon on desktop. Konqueror window comes up.
* In the tree view on the left pane, open "Devices" (click +) then inside that, open \
                "CD-ROM" (click +).
* Open something else in the tree view, e.g. "Hard Disc (hda6) [/]" by clicking on it \
- not its + button, so that the list of files on your CD ROM no longer appears in the \
                right pane.
* Close Konqueror window. (Now no konqueror windows are visible.)
* Try to unmount CD-ROM from right-mouse-button menu on desktop CD-ROM icon.
* I now get this error "umount: /cdrom0: device is busy".
* [Kill the "preloaded" Konqueror process, then retry unmount: Unmount is then \
successful.]

When I did this slowly (while writing the above) it did not fail, but it fails \
consistently otherwise.

Steve
