This patch adds an entry onto the address chooser for *every* email address
known for every contact, rather than just the preferred address. The 'preferred
address' is no longer treated specially.

This also turns off the automatic column resizing before inserting the 'recent
addresses' rows; their names can be very long, which pushes the second column
too far over to the right.

Im not proposing this as a permanent solution. It works for me, for now.
