That is a problem of the mbox-format. You should convert your mailboxes to the 
maildir-format (see the FAQ).

When an abnormal exit occurs in kmail (for any reason, be it Kmail crashing, KDE or \
Xwindows exiting abnormally, or a power off or system crash -- note that any of these \
are rare), messages that I had already deleted from the inbox to the trash are \
forgotten, and when I open kmail again, the messages are not in the trash, but still \
in the inbox.  This is not a major bug, but it does make me have to go through large \
amounts of mail again, as I usually live kmail running for very long periods.  \
