These errors come from the XML parser and indicate that something is
wrong in the file as far as XML syntax. Check the line indicated in the
error message and the answer should be obvious if you know some XML (or
you can post just that part of your doc to this list).

In the first case, the extension of the file in question indicates that
it may not be an XML file.
In the second, '&' and '<' characters have special meaning in XML and
have to be escaped if you want to use them literally, just as in HTML,
like you say. So use the HTML escapes and you'll be fine.
