What we have done with WebWork is use Confluence to generate all our
documentation. Then we use the JIRA macro plugin to include all the
issues. We also have a standard Confluence template for release notes,
making each release consistent:

Note that while we show all the issues, we also have a high level
description of the key changes for the release that is written by
hand. We also do some other neat tricks you  guys might be interested
in, like generating documentation based on code in CVS using a
"snippet" macro that we borrowed from CodeHaus and extended.

All the "yellow content" comes from the source files directly.When the
content is exported, the yellow background and "refresh link" are not
rendered.
