No suggestions as such. Just questions.

Which version are you running?

"/console" is an unexpected pathname. I'd expect /dev/console or 
something like that. Do you actually have a file/directory named 
/console in your filesystem?

You said you've done a failover. On what node do you get the message? 
Has the original initnode rejoined the cluster when you see it?

Do you have any clue just what process is causing this? Is it being 
started by init or service restart or is it something that is trying to 
migrate? (Is loadleveling on?)

I think those'll do for the start.
