If you use smtp protocol, then problem is in your email server and not in
SquirrelMail.

When SquirrelMail uses smtp protocol to send messages, smtp server
confirms that message is accepted.

If you don't provide enough details about your setup, I can only guess.
Maybe you forgot to start email delivery process. Maybe you have used all
partition that stores your emails or exceeded your quotas. Maybe you have
paused delivery. Maybe smtp server thinks that emails are infected and
puts them in quarantine. Maybe something is wrong with your email filters,
tmda or local delivery agent. Maybe something is wrong with dns.

Trace your email from SquirrelMail to delivery location. Your email
server's documentation should have some tips about testing email sending
and delivery.
