Remove the item from where it really is

IntRemoveMenuItem assumes that the item to be removed is directly in the Menu
that it was given, but if the item is being removed with MF_BYCOMMAND, it may
actually be in a submenu. When this happens, it tries to remove the item from
the menu it isn't actually in, and leaves the item pointer in the submenu
dangling.
