It's not possible to implement the desired behavior of toolbar actions, in 
source mode. For example, if bold is toggled, no tag should be inserted in 
the document. Only after the user presses a key, the strong tag should be 
inserted along with a text node. Instead, if the user places the cursor 
somewhere after toggling bold, the bold action should reflect the context the 
cursor are in, and no strong tag should be inserted at all.
