in my installation the default place to save toolbars is:
.kde/share/apps/quanta/toolbars/
In there you will find some folders where the custom toolbar layout is stored
in files like: standard.toolbar.tgz
If you remove the files in your home folder Quanta will fall back to the
toolbars in /opt/kde3
If you can not find the *.toolbar.tgz in your home folder you might have used
Quanta as root user because only then you would be able to save them under
/opt/kde3
I have no idea why the order of the toolbars would change but removing the
custom toolbar is likely to bring the old order back.
